Background & Problem: Nova Scotia Health, the largest provider of health services in Nova Scotia, faced significant challenges that will sound familiar to many Canadian healthcare providers. This led to identifying multiple transformation projects to overcome silos, address inefficiencies, and improve experiences.

A key priority was implementing a common data governance model by migrating hundreds of millions of healthcare data points into a single cloud data store aligned with the International Fast Healthcare Interoperability Resources (FHIR) standard. This created a common framework for managing medical information across systems.

The team then created a web-based application with locations of healthcare services, from hospitals to diagnostic services, urgent care, and pharmacies. This unified relevant information in one place, making it updatable with just a few clicks.

The flagship project was the YourHealthNS digital platform, which launched provincially in November 2023, giving citizens easy access to trusted healthcare information. By July 2024, health records within YourHealthNS became publicly available for every citizen in Nova Scotia aged 16 and older.

The Outcome: Transformative Impact on Healthcare Delivery

As of March 2025, YourHealthNS had been downloaded approximately 630,000 times in a province with a population of just over 1 million. The platform has become a powerful tool for reaching citizens directly and empowering them with better healthcare information and access.

The digital transformation has also improved patient safety and health outcomes. Patients now come to appointments with more understanding of their health conditions, leading to more focused conversations about next steps and care plans. They can share comprehensive lists of medications, vaccines, test results, and other information with their providers, allowing for a more accurate and holistic view of their situation

Making IV Meds Safer & Smarter at Mackenzie Health

Background & Problem: Picture this: a busy hospital ward. Nurses are juggling multiple patients, and many need medications delivered intravenously (IV). At Mackenzie Health's hospitals in Vaughan and Richmond Hill, Ontario, like many places, programming those IV pumps used to be entirely manual. Sounds simple, but it's fraught with risk. Punching in the wrong dose, the wrong rate, selecting the wrong medication from the pump library, or even accidentally programming the pump for the wrong patient – these weren't just theoretical risks, they were real possibilities with potentially serious consequences. On top of the safety concerns, it was inefficient. Manually programming pumps takes time – precious minutes nurses could be spending on direct patient assessment and care. Add manual documentation of infusion details into the Electronic Medical Record (EMR), and you've got a recipe for potential errors and workflow bottlenecks.

The Solution: Mackenzie Health decided to tackle this head-on, becoming the first health system in Canada to implement a specific kind of smart pump technology: the BD Alaris™ EMR Interoperability system, connected to their Epic EMR. Think of it as making the IV pump and the patient's chart talk to each other directly. Here’s how it works: a nurse scans the patient's wristband, the medication barcode, and the IV pump. This triple-check electronically links the specific medication order in the EMR to that specific patient and pump. The system automatically pulls key order details (like drug name and dosage) from the EMR to the pump, pre-populating fields and adding crucial safety guardrails. Crucially, the communication goes both ways. Information like infusion status, alerts, and rates automatically flows back from the pump into the patient’s EMR in real-time. No more manual transcription!

How Four Ontario Hospitals Mastered Digital Transformation

Background: In 2021, four Ontario hospitals—Collingwood General and Marine Hospital (CGMH), Georgian Bay General Hospital (GBGH), Headwaters Health Care Centre (HHCC), and Royal Victoria Regional Health Centre (RVH)—formed the CARE4 partnership. Their mission? To tackle Canada’s fragmented healthcare data ecosystem head-on. By 2024, they achieved HIMSS EMRAM Stage 6 validation, a feat only six other Canadian hospitals had accomplished. This wasn’t just an IT upgrade—it was a full-scale clinical and operational revolution.
